Application process

1. The Sponsor and the Apprentice are responsible for submitting their information to the Apprentice/ Sponsor Jot Form.
2. Apprentice must Apply online for a permit to practice. Once the board receives the notification of your jotform completion, rather than attending an in-person orientation, you will receive a PowerPoint that will allow you to have self-paced training. If approved, the apprentice will receive their license via mail and email, allowing them to start the apprenticeship program. All fees are nonrefundable.
3. The Sponsor is responsible for submitting the Apprentice monthly report labeled Sponsor Certifications for Apprentice Training. 
4. Note: Sponsors are required to resubmit their information every 5 years.
